Haryana : In a first-of-its-kind move, the Haryana government is set to introduce a new model to hire specialist doctors under the National Health Mission (NHM), starting from Panchkula. Instead of fixed monthly salaries, doctors will now be paid on a per-case basis — meaning, they will earn according to the number of surgeries or medical cases they handle.

The pilot project has already been rolled out with three vacancies for specialists announced in Panchkula. The initiative is aimed at addressing the chronic shortage of expert doctors in government hospitals, especially in remote and semi-urban regions of the state.

Under this new model, doctors will work on a contractual basis and receive a fixed amount for every patient case or surgery they perform. This move is expected to create a performance-driven healthcare system and improve the quality of care in government institutions.

Health department officials say that the step will benefit both doctors and patients. For doctors, it offers an additional income opportunity and flexibility, while for hospitals, it ensures the availability of skilled experts when needed.

The NHM will monitor and regulate the hiring, payment, and quality of services. The payment structure will vary depending on the complexity of the procedure — for example, minor surgeries may fetch a lower amount than high-risk operations like C-sections or orthopedic procedures.

Officials are optimistic that this model will attract experienced doctors who were hesitant to work on government terms earlier. By providing performance-linked incentives, the state aims to retain medical talent and reduce dependency on referrals to private hospitals.

If the Panchkula model proves successful, it may be expanded across other districts of Haryana. The initiative is part of the state’s larger goal to strengthen its public health infrastructure and reduce the patient load on overburdened primary health centers.

The government is also exploring possibilities of integrating telemedicine support alongside these specialists, particularly in rural zones. As Haryana embraces innovation in public health, this “per-case payment” system might just be the game-changer the sector needs.
